Travel Nurse

1. Settling at home service

​

Do you need picking up from hospital, a respite care facility, or are you returning home from an extended stay elsewhere? 

​

Our settling at home service provides a combination of nursing and care support, so the transition becomes a smooth and stress-free one for you and your loved ones.

We will decrease the risk of you suffering a relapse and needing to be re-admitted to hospital or institutionalized care.  

 

We manage the following risks:

​

• Medications as prescribed; forgotten, missed and mismanaged 

• Missing medical follow-up appointments

• Poor or lack of nutrition & Hydration needs

• Not following recommended rehabilitation exercises

• Home Hazards Identified for falls risks and independent living difficulties

• Transport difficulties in driving own car or managing public transport services

 Adaptive Nursing offers the following services:

​

• Travel and meet you at a designated place of discharge 

• Manage personal belongings, including luggage, paperwork, medications 

• Overseeing hospital discharge process to ensure all medications are present

• Ensuring post discharge instructions are well understood 

• Transport home

• Reconcile medications prescribed with home medications. 

• Liaison with GP and/or pharmacy

• Procurement of medication regime support tools

Studies show that unintentional changes to medication regimes during hospital admission can cause patient harm during a hospital stay or after discharge. It is estimated that 50% of medication errors occur at transition of care, with one or more medicines missing from discharge information, causing greater risk of hospital re-admission.

At Adaptive Nursing, we can provide the following personal care during a home transition:

• Bathing, showering

• Wound care

• Laundry

• Meal preparation

• Grocery shopping

• Reunion with pets

• Pet feeding regimes

Adaptive Nursing also coordinates follow-up health care services such as:

​

• Welfare checks with approved providers

• Ongoing support from a Registered Nurse - see In Home Nursing 

• Referrals to other health care providers.

• Home safety checks

• Home Assist Secure applications.  

 

Studies show that up to 35% of seniors are re-admitted after only 90 days from hospital discharge due to a lack of proper care. Hospital re-admission can cause delays in healing and create unnecessary pain, stress, and discomfort, while incurring further costs.

 

2. Travel service

​

Most of us have fond memories of our holidays enjoyed as children, teenagers, parents, and grandparents. However, there may come a time in life when thinking about taking a holiday just seems too difficult. It may be due to disability, frailty, or the lack of someone to travel with.

Adaptive Nursing can be your travel companion.

 

We can provide you with a travel nurse who will be your companion for vacations, family reunions, weddings, local excursions, and even international travel.

We can assist in arranging the necessary support, such as having portable oxygen, scheduling medications, assisting with personal care, and navigating access requirements to venues.

We can arrange the necessary comfort and mobility aids (wheelchair, wheelie walker, walking stick) for any mobility difficulties or potential hazards via a mobility equipment provider.

   

We can help arrange accommodation with the appropriate facilities and ensure aids and equipment are secured. We can provide airport transfers and help you navigate airport terminals.

We can also support you while on holiday with medication regimes, wound care, continence care, respiratory care, and personal needs, such as showering, dressing and meals.

The Registered Nurse will provide you with companionship during your trip and can take care of small domestic tasks.
